• $200 per Clinical Intake
• $200 Individual Counseling
• $225 Family/Couple Counseling
• $155 Premarital Counseling
• Please call for international rates and fees
• Phone calls and additional time are billed at $50 per 15 minutes

Insurance

                 Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

We’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?

Tree of Life Counseling and Consulting Services accepts EAP: Employee Assistance Program (EAP) provides pre-authorization information to access counseling services as a paid benefit by your employer. This pre-authorization must be presented at the time of intake/ before the first appointment. Services provided without the current EAP authorization documentation is a cost billed to you directly or through your insurance. No refunds will be provided for collected copays if the documentation is not summitted prior to services being rendered.

Payment

We accept cash, check, and all major credit cards as forms of payment. Please call for sliding scale opportunity.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged for the full rate of the session.
